What to Bring to Your First Appointment
For your first appointment, you will need to bring your medical records, including (but not necessarily limited to) X-ray films, written pathology reports, and pathology glass slides. If you can't carry these with you, please make sure to ask your primary physician to transfer them directly to us a few days in advance of your visit. Without them, we may not be able to complete your evaluation.

Registration
Before your appointment, you will be contacted by a member of our staff, who will request registration information such as your address and health insurance coverage. If you have any further questions about insurance matters, you are encouraged to talk to our staff in the Financial Information Services office at (617) 632-3455.
