Partners Hospice
Partners Hospice is committed to providing premier end of life care to adults and children with terminal illnesses. Their goal is to ensure that comfort and assistance is offered to patients and their loved ones when managing the complexities that come with a life limiting illness.
All Partners Hospice patients receive a collaborative plan of care that is managed by an interdisciplinary team of physicians, nurses, social workers, home health aides, and chaplains. The hospice approach addresses the symptoms of the illness, as well as the patient's and family's emotional and spiritual concerns.
Partners Hospice services are offered in extended care facilities, hospitals and homes in more than 50 communities throughout the Greater Boston and MetroWest areas.
Services offered
- Pain and symptom management
- Assistance with the emotional and psychosocial aspects of dying
- Medication, medical supplies and equipment
- Family coaching on caring for the patient
- Spiritual care
- Special services, such as physical therapy, when indicated
- Transition to short-term inpatient care (if symptoms become too difficult to manage at home)
- Bereavement care and counseling to surviving family and friends
