Our providers work together across a range of specialty areas to provide fully coordinated, personalized care. A dedicated team of clinicians that specializes in your type of blood cancer works with you throughout your treatment, from diagnosis through long-term follow-up and specialized survivorship support. Patients are followed by the same attending physician and nurse practitioner throughout their treatment — enabling continuity of care, a strong patient-care team relationship, and clear and easy access for patients and their local providers.

Patients receive world-class care from a team of specialists, including medical oncologists, radiation oncologists, hematopathologists, infectious disease specialists, hematologists, physician assistants, nurses, oral surgeons, pharmacists, nutritionists, physical therapists, search coordinators, care coordinators, financial counselors, and clinical social workers.

Our entire care team has extensive experience in stem cell/bone marrow transplant. Our specialists work closely together and collaborate regularly to ensure that your care plan offers the best possible outcomes and that all your needs are met. Your team will also be in communication with your local oncologist throughout your treatment and during your post-transplant care.

Oncology Nurse Navigator

The oncology nurse navigator, a key point of contact for clinical issues, works collaboratively with your physician and other team members to coordinate and oversee your treatment plan, and will play a key role in your treatment both before and after transplant. He/she also functions as an educational resource for you and your family.

Stem Cell Transplant Nurse Practitioner/Physician Assistant (NP/PA)

A stem cell transplant nurse practitioner or physician assistant is a mid-level practitioner who will be an integral part of your outpatient care, and in some cases will assist with your care as an inpatient. You will meet with an NP/PA at the time of your first outpatient follow-up visit after transplant. Working closely with your transplant physician, the NP will be responsible for managing your care, including complications, medications, and education.

Stem Cell Transplant Research Nurse

If you are participating in a clinical trial, the research nurse will give you information about the trial, as well as coordinate the plan and requirements of the protocol with your medical care and transplant plans.

Stem Cell Transplant Oral Medicine Team

You must see a dentist and have a dental evaluation before your admission (even if you wear dentures). A member of the Brigham and Women’s Hospital (BWH) Division of Oral Medicine and Dentistry team will review the dental evaluation prior to transplant, as well as routinely monitor your oral health while you are in the hospital.

Your Care Coordination Team

Clinical Social Worker

A clinical social worker can provide counseling and emotional support to you and your family to help you cope with concerns or issues that may arise before, during, or after transplant. He/she can also discuss community resources available; refer to you the Dana-Farber resource specialists, if appropriate; and connect you to additional supports.

Ambulatory Pharmacy Resource Specialist

A pharmacy resource specialist is available to assist you and your caregivers prior to transplant, while you are an inpatient, and after transplant to help identify and address pharmacy assistance needs.

Inpatient RN Care Coordinator

The inpatient RN care coordinator provides care facilitation, discharge planning, and utilization management. He or she will also work with you and your family to plan for your transition from the hospital to the home setting.

Ambulatory Resource Specialist

A hematologic oncology/BMT resource specialist will work with you to identify and help to arrange short-term accommodations (for example, nearby hotels), transportation, parking, and referrals to community-based programs.

Financial Counselor

A financial counselor will provide support and guidance in handling the costs of your transplant care. The counselor's role is to make the financial side of your transplant as stress-free as possible. He or she may also be able to direct you to sources of financial assistance outside of Dana-Farber and Brigham and Women's Hospital.
