How to Transfer a Patient from Emergency Departments or Community Hospitals to Dana-Farber

• Patient Information: name, date of birth, sex
• Patient Diagnosis
• Reason for Transfer

For Pediatric Patients

Call 866-771-5437. Dana-Farber collaborates with Boston Children's Hospital to provide inpatient care for children.
