At Dana-Farber, patients can share important information directly with their care team through pre-visit questionnaires. Responses to these questionnaires give your care team valuable information to support your care. This information contributes to Dana-Farber’s efforts to improve quality of care and has been shown to improve outcomes such as quality of life, survival, and reduced visits to the emergency department.

Patients can complete pre-visit questionnaires online through Patient Gateway or on iPads in Dana-Farber clinics.
